What Is Breast Implant Revision ?

Breast implant revision is a surgical procedure that involves replacing existing breast implants for various reasons, such as natural changes in the body over time, complications, or aesthetic preferences. This procedure also aims to change or improve the appearance of the breasts which helps maintain a harmonious appearance and ensures that results align with the patient’s expectations.

Why considering it ?

Natural Changes in the Body

Pregnancy, weight fluctuations, or simply the aging process can affect the appearance of the breasts. Most often, a sagginess of the breast tissue is observed, where the gland falls below its normal position while the implant remains where it was placed. The upper part of the breasts appears flatter while the lower part is fuller, creating a descending curve reminiscent of a waterfall. This complication is known as 'waterfall deformity.'

Post-Operative Complications

Several post-operative complications may arise, such as infection, the presence of a seroma or hematoma, secondary displacement of the implant, capsular contracture, rupture of the prosthesis, breast implant illness, or anaplastic large cell lymphoma associated with breast implants (BIA-ALCL), which may require surgical intervention.

Desire for Change

Following an initial breast augmentation, many women choose to change their breast implants for various reasons. This could be due to aesthetic concerns, such as the desire to switch to a different size or to modify the shape of the breasts.

How does it work ?

Personalized Consultation

Before the procedure, a consultation with the surgeon is necessary to assess the needs and expectations of the patient. After an open discussion about motivations, preferences, and concerns, the surgeon examines the breasts and establishes a personalized treatment plan. Additionally, information and explanations will be provided regarding the breast implant exchange surgery process, pre- and post-operative instructions, as well as benefits and risks. Depending on the case, pre-operative tests may be prescribed. The breast implant exchange surgery is performed under general anesthesia and lasts between 1 to 4 hours, depending on the complexity of the case and any additional surgical procedures required.

Procedure Steps

The surgery involves several steps: - Removal of Old Implants: Existing implants (intact, cracked, or ruptured) are removed through a discreet incision, often made in the same location as the previous surgery. - Pocket Reshaping/Capsulectomy: Pocket reshaping helps to reposition the implant on the chest whereas capsulectomy refers to the removal of the peri-prosthetic capsule in case of capsular contracture, BIA-ALCL, or any other condition requiring its removal. - Placement of New Implants: New implants, typically made of silicone gel, are then inserted into the old pocket. There is also the option to create a new pocket in a different anatomical plane, depending on the needs and preferences of the patient. - Breast Reshaping: A periareolar or anchor-type mastopexy technique might be necessary in order to correct ptosis, sagginess, and to remove excess skin. - Closure of Incisions: The incisions are carefully closed to achieve the most discreet scars possible. Other surgical procedures such as removal of the peri-prosthetic capsule, mastopexy, or liposuction may be performed based on the patient's needs.

What happens next ?

Immediate Recovery Phase

After the procedure, a surgical bra is generally prescribed to support the healing process and the new implants. Some discomfort is normal, often described as soreness. Pain relief medications may be prescribed to alleviate pain. Mild swelling and bruising are common and gradually subside.

Return to Social Life

A social rest period of 1 to 2 weeks is generally recommended, depending on the extent of the procedure. During the 4 to 6 weeks following the operation, it is advised to avoid intense physical exertion and lifting heavy objects.

Medical Follow-Up and Healing

Follow-up appointments with the surgeon are important to monitor healing progress and ensure that results meet expectations. Scars will fade over time and can be treated with specific creams to improve their appearance.

Final Results

The final results after Breast Implant Exchange typically are expected after 3 to 6 months, when swelling completely resolves and scars fade. The breasts will then appear natural and harmonious matching patient's expectations.

Tips for Optimal Recovery

Avoid direct sun exposure on the scars to prevent pigmentation. Maintain a balanced diet and avoid smoking to promote optimal healing. Follow the surgeon's recommendations regarding the use of the surgical bra and physical activities. Breast implant exchange is a wonderful opportunity for women to adjust and continue to feel confident in their appearance, ensuring that their silhouette remains in line with their desires and lifestyle.
